Please print and fill out the 2011/2012 youth medical/liability form for your child. This is a mandatory BPC form that must be completed once a year. We have new tracking systems in place to ensure that you only have to complete one for the year! Please turn it in to Julie in the church office. Thank you!

How to Be a Driver For A Youth Event
If you are considering being a driver for a Youth Ministry Event you will need to:
1. Provide us with a copy of your current valid California Driver’s License.
2. Provide us with a copy of your current auto liability insurance card. Minimum liability insurance requirements for private passenger vehicles (California Insurance Code §11580.1b) $15,000 for injury/death to one person/$30,000 for injury/death to more than one person/$5,000 for damage to property. We prefer liability limits at $500,000/$500,000.
3. Provide us with your current driving record. You may obtain this report through the DMV office.
4. Provide us with an Authorization to Release Personal Information form. It is BPC policy that all drivers have a background check on file.
